What WOOF! Services are you interested in? Training, Grooming, Wellness, Fit n' Fun Play care, etc...

I hereby release WOOF LLC (WOOF! Wellness Center), its employees and any and all property owners from liabilities arising from my dog(s) attendance at WOOF! Wellness Center.

I understand that dogs are unpredictable and when dogs play in open areas with others dogs that the possibility of injury does exist. I agree to assume all liabilities and expenses resulting from my pet’s actions.

I understand and agree that if in my absence any problem develops with my dog(s), WOOF! Wellness Center and/or its employees will act on my behalf and I further agree to assume all responsibility for any expenses involved.

I understand no refunds will be given on training classes AFTER the end of the first class session.

I agree that the name and likeness of my dog may appear in print, video or on the WOOF! Wellness Center website for promotional purposes.
